Filing a Complaint Against a Facility

If you have concerns with...

  • physical or emotional abuse
  • neglect
  • or unexplained injury

Act quickly. Call the Minnesota Adult Abuse Reporting Center (MAARC) at 844-880-1574 (toll free) or file a complaint online at MAARC Mandated Reporter Form.

If you are a mandated reporter and/or provider who has a concern regarding a vulnerable adult family member or concern regarding another provider, place your concern through the MAARC system.

Prompt Action is Vital

  • Prompt action is vital, as there may be time-limits on what can be investigated.
  • Write down all you know about something you think may be a violation, such as location, names, times, etc.
  • Call the above numbers.

Filing a Complaint for a Minor Child (Younger than 18 Years Old)

If you have a concern regarding a minor child, contact the county where the child lives. See Minnesota Department of Human Services: Minnesota's county and tribal child protection agencies.

Filing a Complaint that Does Not Include Physical or Emotional Abuse, Neglect, or Unexplained Injury for Adult or Minor Child (Younger than 18 Years Old)

If you have a concern regarding a vulnerable adult that is not related to physical or emotional abuse, neglect or unexplained injury, or financial exploitation contact the Office of Health Facility Complaints at 651-201-4200 or health.ohfc-complaints@state.mn.us.
