Housing with Services Establishments/Assisted Living Designation & Uniform Consumer Information Guide
NOTICE: As of Aug. 1, 2021, housing with services (HWS) establishments statutes will be repealed. HWS registrations will no longer be valid. Per 144G.191, HWS registrations will not be accepted after May 31, 2021. If you are interested in assisted living licensure, please reference MDH’s Assisted Living Licensure webpage for more information.
Please do not use these forms to renew a current registration. Currently registered establishments will receive separate notification and instructions for renewal from the Department of Health. If you are unsure this is the correct registration for your business or have questions, please contact the Licensing and Certification Program for additional clarification.
