Sherburne Community Health Assessment and Planning

Priority health issues (2025)

Benton, Sherburne, and Stearns counties, along with CentraCare, developed a partnership called the Central MN Alliance and share a community health assessment and improvement plan.

Central MN Alliance noted the following priority health issues in 2025:

• Healthcare access
• Substance use
• Mental health
• Economic stability
• Social connectedness

Community health improvement plan

A community health improvement plan guides a community health board and its community partners on work to improve the health of its population.
