Abortion Reporting System

Abortion Reporting System

This secure web-based abortion reporting system was launched in March of 2017 as a module of the Minnesota Registration & Certification system (MR&C).

To comply with statutory requirements for reporting abortions in Minnesota, three reporting forms are in use. The first form, Report of Informed Consent Related to Induced Abortion, captures data about the information that must be given to the potential patient at least 24 hours prior to an abortion procedure. The second form, Report of Induced Abortion, captures information about the patient and the abortion procedure. The third form, Report of Complication(s) From Induced Abortion, captures information on any complication resulting from an induced abortion that is diagnosed and treated by a physician at a later date. All three forms may be submitted via this web-based reporting system.

Facilities filing many reporting forms per year will be required to use this system to submit their reports electronically. However, those physicians, clinics or hospitals who have few reports to submit each year may instead complete a paper copy of the form and mail it to the Minnesota Department of Health for entry by department staff.  Links to the reporting forms are below. These are fillable PDF forms and may either be completed in the PDF document and then printed or they can be printed first and completed by hand.
