What to report

Pulmonary or extrapulmonary sites of Tuberculosis (Mycobacterium tuberculosis complex), including laboratory confirmed or clinically diagnosed tuberculosis disease, must be reported to MDH within one working day.

How to report tuberculosis

MDH requests that tuberculosis cases be reported by phone or fax (i.e., not by mail) to help ensure timely public health follow-up measures.

  • Phone
    Any reportable infectious disease may be reported by phone to 651-201-5414 or 877-676-5414.

  • Yellow Disease Report Card
    This disease may be reported using the MDH "Yellow Card."
    Tuberculosis reports should be faxed to 651-201-5500.

Who is required to report

  • Health care practitioners (health care facilities, medical laboratories, and in certain circumstances veterinarians and veterinary medical laboratories) are required to report disease to the Minnesota Department of Health (MDH) under Minnesota state law.
    • Unless previously reported, every licensed health care provider who provides care to any patient who has, is suspected of having, or has died from a reportable disease is required to report.
  • Any person in charge of any institution, school, child care facility, or camp is also required to report disease to MDH.

Submitting clinical materials

For tuberculosis, submission of clinical materials (isolate, if available) to MDH is required by rule.

TB: Special Reporting Requirements

  • If a person required to report has reason to believe that a person with active pulmonary tuberculosis refuses treatment for active tuberculosis, or has not complied with prescribed therapy for active tuberculosis MDH should be notified one working day (with the name, address, and essential facts of the case).
