If you have already submitted a Department of Human Services Applicant Background Study (PDF) for your licensing prior to Oct. 1, 2018, you do not need to pay MDH a required one-time background processing fee until your renewal, after Jan. 1, 2020, per Minnesota Statutes, section 148.5194, subdivision 7a.

Criminal background check processing fee
*A one-time processing fee of $13 is collected by MDH to cover costs associated with a Department of Human Services (DHS) criminal background check. The fee table above includes that cost in the total. Background check fees paid to DHS are additional.

Hearing instrument dispenser exam
