Medical Cannabis Materials and Forms
Educational Materials
NOTE: As a convenience to our stakeholders, the Office of Medical Cannabis is offering several new fact sheets available for download directly from our website. In addition, we streamlined information in the patient guide into one brochure. Brochures focused on patients and caregivers will still be available for printing, but other brochures will not. We have redesigned those materials into one-pagers to better support program outreach.

Surveys
The Office of Medical Cannabis currently employs three surveys that are administered via the Medical Cannabis Patient Registry. These surveys give patients and their certifying health care practitioner to give us critical feedback on the patient’s experience with the program.
