2025-2030 Minnesota Healthy Brain Community Grantees
The Minnesota Healthy Brain Community Grants program is excited to announce the recipients of the 2025-2030 funding cycle. The Healthy Brain Community Grants support organizations that reach and engage communities in Minnesota impacted by Alzheimer's Disease and related dementias to improve brain health.
The purpose of this funding is to increase early detection and diagnosis of dementia, provide culturally responsive education and awareness about dementia risk reduction and the benefits of early diagnosis, and enhance wellbeing for people living with dementia and their care partners.

Centro Tyrone Guzman
This grant will fund Centro to curate 5 types of Montessori-influenced Healthy Brain boxes for Latine elders. In Year 1, Centro will with Latine elders to produce materials for the first set of Healthy Brain Box and disseminate the boxes to older adults in the community. Through the Healthy Brain Boxes, Centro's goals are to raise awareness about dementia risk factors, the importance of early detection and diagnosis, and support caregivers with resources specific to the communities they serve.

Fellowship Missionary Baptist Church
The funds from this grant will support the development of a Cultural Care Village that aims to educate and inform caregivers about brain health and dementia. Fellowship staff will engage with families to increase their involvement in the Cultural Care Village to reduce social, community, and spiritual isolation for older adults.

Minnesota Indian Women's
The Healthy Brain Community Grant will support Minnesota Women's Resource Center (MIWRC) to increase knowledge about brain health and dementia among MIWRC program participants through series of community listening sessions led by American Indian elders.

Prosperity for New America
Prosperity for New America (PNA) will identify and recruit a New American Advisory group to guide their work on dementia education and brain health. PNA plans to offer trainings and support advisory group members to do community education and outreach.

Rochester Clinic
This funding will support Rochester Clinic and Lotus Health to host 3 educational community conferences that focus on the impacts of lifestyle changes on dementia and brain health. Rochester Clinic will engage with older adults through post-conference activities such as sound bowl meditation and cooking demonstrations. By strengthening partnerships in the area, Rochester Clinic will explore different ways of how they can build community-clinical linkages.

Resource - Minnesota Dementia Strategic Plan
The Minnesota Department of Health (MDH) along with the Minnesota Healthy Brain Partnership (MNHBP) developed a Minnesota Dementia Strategic Plan (PDF). The MNHBP includes organizations that work on issues related to dementia and meets to advance this important work.
The plan is organized into four main areas:

- Develop Policies and Partnerships
- Ensure Culturally Responsive Caregivers
- Accountability, Monitoring and Evaluation
