Background
Starting in 2022, the Health Promotion and Chronic Disease Division completed a strategic planning initiative to develop and implement a vision, mission and set of strategic goals and strategies. This initiative involved engagement with diverse internal and external partners and now is undergoing implementation and monitoring. This plan has been designed to align with agency’s strategic plan and health equity priorities.
Vision
In Minnesota, all people are equipped and empowered to enjoy healthy lives.
Mission
HPCD works collaboratively to advance health equity, promote healthier communities, and prevent and reduce impacts of chronic disease and related disabilities. HPCD addresses risk and protective factors for individuals and communities through policy, systems, and environmental change.
Goals and key strategies
After an extensive planning process with input from a wide variety of partners and key stakeholders, HPCD developed the following goals to guide their work. Goals are written as outcomes and look out 10-15 years, and key strategies are generally for 3-7 years. Hundreds of shorter-term supporting actions were drafted and are the basis for section- and project specific action plans.

Implementation
HPCD is currently working with partners to implement the new Strategic Plan. Some examples include:
- The Stroke Program is a leader in informed, effective programs by ensuring quality stroke care to more Minnesotans through the Minnesota Stroke System.
- The Sage Breast and Cervical Cancer Screening Program works together to advance health equity by partnering to improve cancer screening in Southwest Minnesota.
- The Statewide Health Improvement Partnership (SHIP) works with local strategies, systems, and policies to promote health and healthy equity.

Internally, HPCD has adopted new work groups and collaborative teams to advance key initiatives highlighted throughout the strategic planning process.
- A new community-clinical linkages work group collaborates to reduce barriers and silos for effectively working across clinics and communities.
- The data modernization work group is improving data access, use, and sharing to drive actionable, meaningful, and measurable improvements in chronic disease outcomes across communities.
- Communicators working across several health specific programs now work as a team to address gaps, share expertise, and build more capacity for effective health communications.
Partners
We have engaged a broad range of key internal and external partners connected to our work, including HPCD staff, task forces and committees, advisory bodies, partner agencies and organizations, local and Tribal health entities, statewide and community associations, and others.
