2020 deliverables: 2015-2019 local public health assessment and planning cycle
2019 LPH Act annual reporting (finance and staffing) and submission of priority health issues are due Friday, November 6. Submit what the priority health issues were on December 31, 2019 (pre-COVID), as determined by your CHA-CHIP process with community partners. To submit the priority health issues, follow the instructions in the guidance document immediately below.
We are not requesting submission of the 2020 deliverable documents (CHIP, plan to monitor and revise the CHIP, strategic plan, plan to monitor and Revise the strategic plan) at this time. Community health boards that would like to submit their documents can still do so following the submission instructions, which you can also find in the guidance document below.

- Strategic plan
- Description of how you monitor and revise your strategic plan: A description of how the community health board will monitor and revise the strategic plan over the next five years
- Priority health issues: Up to 10 priority health issues identified by the community health assessment
- Community health improvement plan
- Description of how you monitor and revise your community health improvement plan: A description of how the community health board will monitor and revise the CHIP over the next five years
