How to submit your plans to MDH
At least every five years, Minnesota's community health boards are required by statute to submit a community health assessment and community health improvement plan to the Minnesota Department of Health (Minn. Stat. § 145.04, subd. 1a).
MDH will collect health priorities (determined by a community health assessment process) and community health improvement plans on a rolling basis moving forward. Each community health board will submit these every five years, based on the date the community health assessment and plan were approved, enacted, or adopted (as determined by the community health board).
