CHA-CHIP Community of Practice
Join the MDH CHA-CHIP Community of Practice to build capacity around conducting, developing, and implementing a community health assessment (CHA) and community health improvement plan (CHIP).

Purpose
The CHA/CHIP Community of Practice will:
- Support local public health departments throughout the entire assessment and planning process
- Foster learning about best practices around community health assessment and planning
- Serve as a statewide peer network to encourage troubleshooting, resource and knowledge sharing related to assessment and planning
Format
MDH will hold webinars every other month on a variety of topics related to the community health assessment and the community health improvement plan. These webinars will alternate months with the QI Community of Practice.
In 2018, the CHA-CHIP Community of Practice met in June, August, October, and December.
Who Should Participate?
MDH encourages directors, CHS administrators, planners, and anyone else working on a community health board's assessment or plan to join the CHA-CHIP Community of Practice.
