Registration for Boarding and Lodging Establishments or Lodging Establishments Providing Special Services
As of August 1, 2022, providers holding a valid license from a health-related licensing board (HLB) who has also undergone a background check under Minn. Statutes 214.075, shall not have a background study completed by the commissioner of human services (a NETStudy 2.0 background study).

This page contains information and forms required to register as a Boarding and Lodging Establishment or Lodging Establishment Providing Special Services in the State of Minnesota.
NOTE: Your establishment must first be licensed as a Boarding and Lodging or Lodging Establishment in order to register as providing special services. Information on these licensing requirements can be found on the Minnesota Department of Health (MDH) Environmental Health Division's website at Food, Beverage, and Lodging Licensing.
To determine whether you must register as an establishment providing special services you should review Minnesota Statutes 157.17 and the information at: Boarding and Lodging with Special Services.

Required Forms and Documents
If you have determined that your Boarding and Lodging or Lodging Establishment will be providing special services and must be registered, you should download and complete the registration form: Lodging Establishments Providing Special Services Registration Form (PDF).
Please do not use this form to renew a current registration. Establishments that are currently registered will receive separate notification and instructions for renewal of their registration from the MDH Health Regulation Division.
