Manage Immunizations Frequently Asked Questions

Clients should provide acceptable proof of their immunizations for vaccines not administered by your organization. Below is a list of acceptable sources.

• An official paper or electronic document from a healthcare provider or another state's immunization information system (IIS). The vaccine dose should include the day, month, year, and vaccine administered, as well as the patient's name and date of birth.
  • This can include documentation from a clinical application such as MyChart or similar platforms.
  • This can also include handwritten records on official letterhead or templates from organizations that do not print typed records.
• An official Centers for Disease Control and Prevention (CDC) COVID-19 vaccination card.
• A U.S. Department of State's Vaccination Documentation form DS-3025, which includes the patient’s verified past immunizations.
• An official International Certificate of Vaccination or Prophylaxis (yellow card).

Select the pencil icon in the Edit column of the immunization you need to add a reaction to. Check the box next to the applicable reaction. Click the Save button.

This occurs because your organization is using the inventory function. Look at the “Active immunization inventory on [date]” section.

If the lot number is not in your drop-down list, this vaccine may not be in your MIIC inventory. For more information, refer to Manage Vaccine Inventory.

There are multiple reasons you may not be able to edit an immunization:

• Your user role does not allow you to manage immunizations.
• You are not acting on behalf of the location that reported the immunization.
• Some fields cannot be edited manually by anyone. These include manufacturer, expiration date, and (in some cases) lot number.

You may not be accessing MIIC on behalf of the location that reported the administered immunization. If you do not have access to switching to the appropriate location, contact your health system’s MIIC Administrator for additional help.

MIIC stores data from many provider organizations. If an immunization is incorrect, you need to contact the organization that reported the immunization in question and request that they correct it. If you cannot get in contact with that organization, contact the MIIC Help Desk.

1. Ask the family to contact the child’s healthcare provider and request the healthcare provider report the immunizations to MIIC.
2. If the healthcare provider cannot report immunizations to MIIC, securely send the record to the MIIC Help Desk after receiving consent from the parent/guardian to share the immunization information with the Minnesota Department of Health.
    

There are multiple ways to print the record. From the Immunization History screen, select:

• Print button: Prints a version of the webpage you are viewing.
• Print Confidential button: Prints a version of the webpage you are viewing, without the client’s mother’s maiden name, address, phone number, or chart number.

From the client information screen or immunization history screen, select the Reports button and choose from the following reports:

• Complete Immunization Report: Prints a client’s immunization record with additional immunization details.
• MIIC Immunization Report: Prints a client’s immunization record with client comments and forecasted immunizations.
   

This report can be used to document an immunization your organization administered, for your internal records. You should still report the vaccine to MIIC by using your normal processes. Refer to How to Exchange Data with MIIC for more information.
