Public Use Files

Health Care Quality Measures

The Quality Reporting System Public Use Files (PUFs) are a collection of stratified quality measure data focusing on geographic, race, Hispanic or Latino ethnicity, preferred language, health insurance type, and gender breakdowns of the data. These PUFs are available to users for free.

First published in 2019, the Quality Reporting System PUFs were created following recommendations by members and representatives of communities disproportionally impacted by health inequities and community-based organizations. This partnership was in collaboration with Voices of Racial Justice and focused on data strategies to provide data and companion materials to communities and organizations that serve and represent them. MDH continues working with partners to use and learn from these data and discuss solutions to health inequities across the state.

Description of Available Public Use Files

Data for the quality measures included in the Public Use Files (PUFs) come from Minnesota physician clinics. PUFs are aggregated at the patient ZIP Code area and county levels. PUFs are available for six quality measures. Each measure is focused on a specific medical condition or screening for a specific condition.

  • Adolescent Mental Health and/or Depression Screening
  • Asthma Education and Self-Management
  • Colorectal Cancer Screening
  • Optimal Asthma Control
  • Optimal Diabetes Care
  • Optimal Vascular Care

How to Access Public Use Files

Request one or more of the PUFs by completing the Quality Reporting System Public Use Files request form.

After completing the form, you will be automatically directed to the download page.

Questions may be directed to health.sqrms@state.mn.us.

We Want Your Feedback

Data users are encouraged to send comments, questions, ideas for improving the PUFs, or suggestions for new PUFs. Please email your feedback to health.sqrms@state.mn.us.
