Past opportunities

The Innovating from our Hearts to a Healthier Rondo grant application period closed on Oct. 30, 2024. These projects aim to connect community and clinical services among Minnesota adults in St. Paul’s Rondo and Capitol Heights neighborhoods to improve heart health and reduce impacts of heart disease, hypertension, and hypercholesterolemia.

The Creating Community Linkages to Heart Health grant application period closed on June 12, 2024. These projects connect clinical and community services to improve cardiovascular health and reduce impacts of heart disease, stroke, and diabetes among Minnesota adults.
