Stroke Program Funding Opportunities

Stroke Team-Based Care: Request for Proposals

The Minnesota Department of Health (MDH) Stroke Program is requesting proposals from qualified organizations for innovative solutions to enhance the use of team-based care to improve communication, coordination, and collaboration in the care of stroke survivors and individuals at risk for stroke.

Important dates

  • Last day to submit RFP questions: Monday, October 6, 2025
  • Applications due: Wednesday, October 15, 2025
  • Estimated date applicants to receive award and denial notice: November 2025
  • Grant Start Date: Friday, January 2, 2026

Funding

MDH estimates two grantees will be awarded $15,000 each for one (1) year. This grant opportunity is a one-year grant with the possibility to extend two years based on funding availability.

Online Stroke Team-Based Care Application: Contains program overview, scoring criteria, application questions, workplan and budget. You will also be able to upload Due Diligence, Conflict of Interest, and Letter(s) of Support if applicable.

Applications must be submitted into REDcap Stroke Team-Based Care Application by Wednesday, October 15, 2025.

Please submit questions no later than 4:30 p.m. Central Time on Monday, October 6, 2025
