Learning and Innovation Work Group
Charter

Objective: To recommend HCH program improvements with consideration of learning, innovation, best practice, and alignment with other state and national initiatives and care delivery models.

The Learning and Innovation Work Group makes recommendations to the HCH Advisory Committee on:

  1. Development of operational aspects of the progression model in anticipation of rule changes
  2. Criteria proposed to meet certification and recertification requirements and possible additional levels of certification
  3. Value of activities, learning, and tools to support capacity building for certification and recertification and advancing the HCH model
  4. Strategies for inclusion of patient, family and consumer perspectives
  5. Efforts to improve the health of all Minnesotans and advance health equity through team-based, patient-centered care approaches that address social determinants of health
