Advisory Committee

The Health Care Homes Advisory Committee is composed of a broad range of stakeholders, including primary care providers; mental health providers; care coordinators; certified Health Care Home clinics with statewide representation; health plan companies; state agencies; employers; academic researchers; consumers; and organizations that work to improve health care quality in Minnesota.

The Advisory Committee meets two to four times per year and has the authority to establish work groups as necessary to address specific topics or issues.
