Track attendance for the PHN residency program

By completing the monthly activities in the new graduate residency program, both residents and preceptors are eligible to receive a certificate of completion.

These activities may be eligible for continuing education credit. Each nurse is responsible for determining whether each component meets the requirements for acceptable continuing education hours.

Evaluations for this program are scheduled at 3, 6, and 12 months. After submitting each evaluation, you will receive an email with a certificate of completion to be used for continuing education.

Track your attendance and component hours: Attendance tracking form for preceptors and residents (DOC)
