Data presented below is for all cases, regardless of interview status. Data for cases pending interview may be listed as "unknown/missing."

Positive PCR test results are considered confirmed cases. Positive antigen test results are considered probable cases.

All probable cases get the same public health follow up and recommendations as cases confirmed by PCR tests.

Likely Exposure is defined by a hierarchy of exposure types. The order of likely exposure categories is:
  • Community (outbreak): Case was exposed to a known outbreak setting in Minnesota that is not also a congregate living setting (e.g., long-term care, corrections, shelter) or health care setting. This includes restaurant/bars, sports, worksites that are not living settings, etc.
  • Travel: Case traveled outside of Minnesota in the 2 weeks before illness.
  • Congregate Care Setting: Residents, and staff who are not part of a non-congregate care setting outbreak and did not have an exposure to a positive household member. Congregate care settings include long-term care facilities (LTCF), assisted living facilities, group homes, or residential behavioral health (RBH) facilities.
  • Corrections: Inmates who were exposed while incarcerated, and staff of a jail/prison setting who are not part of a non-corrections outbreak and did not have an exposure to a positive household member.
  • Homeless/Shelter: Residents/guests, and staff who are not part of a non-shelter outbreak and did not have an exposure to a positive household member.

    This is a cumulative list of long-term care facilities (nursing facilities and assisted-living type facilities) that have reported a positive PCR test (confirmed case) since March 2020 or antigen test (probable case) for SARS-CoV-2 since September 2020 in a resident, staff person, or visiting service provider. This file is updated weekly on Thursdays.
    • This list does not identify the size of the facility — larger settings will likely have higher numbers; facilities of 10 or fewer residents are not included.
    • Staff case counts include staff and visiting service providers that tested positive for COVID-19 and worked within the facility while infectious.
    • Resident case counts include residents that were admitted to the facility already positive for COVID-19, as well as facility-onset cases. LTC facilities that have opened COVID-19 units and admitted COVID-19 positive residents will naturally experience higher numbers and deaths as they respond to the needs of the community and hospital discharge needs.
    • Numbers include confirmed and probable cases and COVID-19 listed on a death certificate or clinical history/autopsy findings that provide evidence that the death is related to COVID-19 without an alternative cause (i.e. drowning, homicide, trauma, etc.).
